If you change the medical system to make being uncon longer or need a medivac/casevac then you are making it a requirement. look at the whole picture.

The issue is, if we set something like that up, where medivac/casevac is required, then we need someone online, running that role 24/7. If we can't make that happen (which I think we can agree we cannot) it just is not feasible. sometimes limitations of this being a GAME mean that realism...
